Community Health Needs Assessment
The Community Health Needs Assessment (CHNA) is an in-depth report that engages community-based organizations, health care institutions, and community members to identify and prioritize the current needs of our community. The Health Project facilitates multiple focus groups and distributes several surveys to ensure we’re doing all we can to listen, understand, and support initiatives that improve the communities in which we serve.
Community Impact Grants
The Trinity Health Muskegon, Shelby and Grand Haven Community Impact Grants program, comprised of the Community Benefit Board Initiative and the Sister Simone Courtade Fund, was created to support, strengthen, and sustain Trinity Health's organizational mission. We aim to promote optimal health for poor and vulnerable populations and communities by addressing social needs, connecting social and clinical care, and ensuring care for the common good.
Over the last three grant cycles, and continuing forward, efforts have intentionally aligned with our neighbors' voices as heard through the Lakeshore’s Community Health Needs Assessments. These have been operationalized along the lakeshore through many community partnerships and collaborations through our Community Health Improvement Strategy Plans.
As our services move forward to better serve our West Michigan neighbors, we are streamlining the support we are offering our community partners. Beginning in 2026, the Community Impact Grant program will be instituting a new timeline that follows the calendar year. The upcoming grant awards cycle will be focused on the following aspects: funding new innovative projects, supporting community organizations, developing enhanced and/or expanded programs aimed at addressing Community Health Needs Assessment priority areas and operationalizing true collaboration strategies over diverse systems of delivery.
